• Industrial Metal-Fab Inc — South Bend
  A multi-generation, family-owned operation dating to 1964. It combines metal fabrication with large-part CNC machining and is geared toward complex industrial parts, welded assemblies, and production work.
• M & S Industrial Metal Fabricators — Huntington
  Family-owned with a broad manufacturing offering: laser cutting, turret punching, forming, shearing, MIG/TIG/spot welding, powder coating, assembly/kitting, and engineering support. This looks particularly relevant if you're looking for a supplier rather than a general welding shop.
• Gammons Metal & Manufacturing. Co. Inc. — Indianapolis
  Founded in 1968, with capabilities spanning prototype through production runs, including laser cutting, punching, CNC stamping, press-brake forming, shearing, and sawing.
• G&G Fabrication and Welding — Westfield
  Family-owned and particularly interesting for custom industrial/structural fabrication and certified welding. It appears better suited to custom assemblies and fabricated structures than high-volume sheet-metal production.
• Indiana Custom Fabrication, Inc. — Hope
  A smaller custom manufacturer with engineering, laser cutting, welding, powder coating and assembly. Its emphasis on returnable steel shipping racks could make it especially relevant to manufacturers needing material-handling or production-support fabrications.
• SAMCO, Inc. — southern Indiana
  Family-owned and operated for more than 50 years. It offers stamping, laser cutting, welding, painting, assembly, prototyping and low-volume production, and serves industries including transportation, defense, agriculture, appliances and electrical products.
• CB Fabricating — Anderson
  A locally rooted precision sheet-metal manufacturer with design assistance, fabrication, assembly and logistics capabilities. A good candidate to investigate for repeat production rather than just one-off fabrication.
• Midland Metal Products — Hammond
  A particularly interesting one for higher-volume manufacturing. It's a family-run business established in 1923, producing wire, sheet-metal and tube products, with assembly/packaging capabilities and ISO 9001:2015 certification.
• Lionshead Precision Metals — Greenwood
  Part of family-owned Lionshead Industries. Its capabilities include deep-draw stamping, laser cutting, close-tolerance machining, forming, welding and assembly, with a 130,000-square-foot Greenwood facility.
• Huncilman Metal Fabrication — New Albany
  A long-established Indiana operation whose ownership is now shared between two fifth-generation family businesses, Huncilman and Nix Industrial. It offers laser cutting, precision forming, welding and in-house E-coat, making it a substantial option for production manufacturing.

A few smaller local shops

If your definition of “locally owned” is more important than scale, Northwest Indiana Fabrication, Fasttimes Fabrication, Phi Manufacturing, and Paramount Metal Fab are also worth a look. They explicitly identify themselves as family/local or owner-operated businesses and offer various combinations of laser cutting, forming, welding and custom fabrication.

One particularly notable point: “family-owned” doesn't necessarily mean small in Indiana. Huncilman/Nix, Midland, SAMCO and Lionshead have family ownership while operating at a scale much closer to conventional manufacturing suppliers.
